The FY 2012 CSBG IS Survey Forms have been updated and posted on the NASCSP website. Most updates involved small changes to formatting and labeling in Section F and the NPIs.

New items to note include:

•           Section C #8 CCAPs and ROMA trainers at the State level.
•           Section F two new items were added: #4.h AFI and #7.a WIA.
•           Section G #16.e is a text box for describing “Other Housing.”
•           NPI 1.1 description text.
